Student Health
Students entering school will not be allowed to attend school until they are properly immunized. If your child has already received these immunizations, please provide their most current immunization record.
Preschoolers
Colorado State Regulations require all preschool students to have records of a current physical and immunizations on file in the school office. At the time of registration, the parent/guardian must provide a physical form and immunization record for each child entering preschool, signed and dated by an approved health care professional. Students entering preschool must have the following immunizations before they can start school:
- 4 DTaP – Diphtheria, Tetanus, Pertussis
- 3 IPV- Polio
- 3/2/1 Hib – Haemophilus Influenzae type B
- 1 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B
- 1 Varicella – Chickenpox vaccine or history of disease provided by doctor/health provider
- PVC (Pneumoccoccal Conjugate) 4/3/2 doses are required, depending on student’s current age and the age when the 1st dose was administered.
Grades K-5th
Colorado State law requires that all students entering Kindergarten in 2013-14 must have the following immunizations before they can start school:
- 5 DTaP – Diphtheria, Tetanus, Pertussis (5 doses of DTaP vaccines are required unless the 4th dose was given on or after the 4th birthday, in which case only 4 doses are required.)
- 4 IPV – Polio (4 doses of Polio vaccine are required unless the 3rd dose was given on or after the 4th birthday, in which case only 3 doses are required.)
- 2 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B
- 2 Varicella –2 Chickenpox vaccines or history of disease provided by doctor/health provider
Grade 6th-12th (Tdap required)
Colorado State law requires all students entering 6th through 12th grades to have the Tdap in order to attend school in the 2012-2013 school year.
- 1 Tdap (Tetanus, diptheria, pertussis) vaccine (6th-12th grade)
- 5 DTaP – Diphtheria, Tetanus, Pertussis (5 doses of DTaP vaccines are required unless the 4th dose was given on or after the 4th birthday, in which case only 4 doses are required.)
- 4 IPV – Polio (4 doses of Polio vaccine are required unless the 3rd dose was given on or after the 4th birthday, in which case only 3 doses are required.)
- 2 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B
- 2/1 Varicella – Chicken pox vaccine or history of disease. 2 Varicella required for 6th and 7th graders, 1 Varicella required for 8th, 9th, 10th, 11th and 12th graders.)

The summer break is a great time to get the required shots for the upcoming school year. Please make arrangements for your student at their health provider.
The Westminster Clinic of Kids First Health Care (303-428-4384) located at 8030 Irving Street, offers a Walk-in Immunization Clinic every day from 8:30 to 4:30. Expanded immunization clinic hours are offered during the summer. Please call Kids First Health Care for additional information. The following requirements apply only to the immunization clinic:Tri –County also offers immunization clinics. Call 303-451-0123 for more information.
- Children must be at least 4 years old
- Children are uninsured or have Medicaid
- Cost is $14 each, due at time of service
- Must bring immunization record
